Students may apply for multiple programs at the same time within this form. However, students should review the requirements for entry into each program prior to completing the application.

About the Application Process:

Apply to 51视频

All limited-entry program applicants must have a current application on file at 51视频. This means applicants are either attending 51视频 currently or have applied to attend an upcoming semester.

Applicants who are not currently attending 51视频 must Apply to 51视频 prior to completing the Health Sciences & Wellness Programs Application. This will assign the 51视频 Student ID number and provide access to submit official transcripts from any previous college attendance. The official transcripts are required in order to award credit for coursework from institutions other than 51视频.

Application Guidance

The HSW Programs Application offers a SAVE option and can be completed in one or several sessions. This allows all applicants to gather required information and ensure their application is complete when submitted. The limited-entry programs have application requirements that may vary slightly.

Jan. 5, 2026 applications open @ 8 a.m.
Feb. 6, 2026 applications will close at 11:59 p.m.
Mar. 9, 2026 application scores will be finalized by the HSW team
Mar. 13, 2026 letters will be sent to students from the HSW team, to the email address provided in the application
Mar. 27, 2026 confirmation of acceptance or declination must be submitted using the link provided in the letter
Apr. 3, 2026 accepted students will hear from their Program Director with further directions

Essay Questions

Some programs require original responses to five essay questions. You will be required to answer these questions one time only, and only if they pertain to your programs of interest. The questions are listed below in order to help you prepare clear and concise responses to each question. Your responses will be limited to 1,000 characters, or approximately 170 words.
